Community Outreach & Home Base Care

Community Care Givers Programme:

We have a group of Home Based Care workers who do home visits out in the community, identifying families in need, assisting with health checks, the wellness of the families, and any domestic issues. We do referrals to social workers for any issues that are found. We also assist with things like getting documents at home affairs for children.

We also focus on the elderly who cannot make the journey to the centre, with our home based care, we are able to look after these elderly people in need which includes their health and well-being.

Community Outreach also includes distribution of Chronic Medication to those who cannot get to the centre. We have been involved in very large projects including that of CASIPO where we distributed HIV/AIDS and Chronic Medications to community clinics surrounding the centre.

Social Development

We at 1000 Hills continue to work closely with the department of social development on HIV and AIDS programmes. We have 29 community care givers (CCG'S) and 4 supervisors who are receiving a monthly stipend from the department. The CCG'S work 5 days a week doing community outreach home-visits, identifying orphans and vulnerable children, assisting those who are sick or in need of a community care giver. Each CCG is entitled to do 3 home visits per day resulting to 15 home visits a month each. The CCG'S meet at the centre monthly where they submit their monthly reports to the department of education.

We are pleased to say that the department is giving us 20 monthly food parcels to give to the destitute families and vulnerable children as well as school uniforms and monthly contributions to our expenses.
